Defects are now reported using the categories and types defined as defined in the purple book.

A comment may also be included by the inspector to provide any additional or more specific information regarding the defect found.

Overview of the Categories and Types to choose from when creating a defect

	Ravelling	Progressive loss of binder & aggregate chippings from the pavement surface. Common on flexible surfaces.
	Bleeding	Fatting up or excess bituminous material on the pavement surface. Common on flexible surfaces
	Wear and Polishing	Worn surface from traffic wearing off the surface mortar. Common on concrete surfaces
	Pop-Outs	Individual pieces of large aggregate popping out of the surface. Common on concrete surfaces
	Scaling	Loss of fine aggregate and mortar. More extensive scaling can cause loss of larger aggregate. Common on concrete surfaces
d. Pavement Deformation	Rutting	Permanent longitudinal deformation in the wheel path caused by traffic loading. Common on flexible surfaces.
	Surface Distortion	Permanent surface deformation (excluding rutting). Common on flexible surfaces.
	Faulting	Difference in elevation across a joint or crack. Common on concrete surfaces.
e. Cracks	Alligator Cracking	Series of interconnected cracks forming small, sharp angled polygons ranging in size from about 25mm to 125mm resembling chicken wire. Common on flexible surfaces.
	Edge Cracking and Breakup	➢Singular/multiple cracking within 300mm of the pavement edge, or ➢Pavement edge showing considerable breakup, or ➢Breakup of pavement edge including any mix of potholes, cracking & patching along the pavement edge All common on flexible surfaces
	Induced Cracking	Cracking of existing road due to trench work
	Other Cracking	➢Longitudinal cracks parallel to centreline - Common on all surfaces ➢Transverse cracks at right angles to the centreline. Common on all surfaces. ➢Reflective cracks from pavement base. Common on all surfaces. ➢Slippage cracking of the road surface - Common on flexible surfaces ➢Meander cracks are cracks that appear to wander randomly. 	Corner Breaks	Diagonal cracks near the corner of a concrete slab, forming a triangle with a longitudinal and transverse joint - Common on concrete surfaces
	Shattered Slab	Slabs divided into 4 or more pieces with severe damage/faulting/spalling along the crack edges. Common on concrete surfaces.
f. Surface Openings	Potholes	Bowl shaped depressions where part of the pavement has been removed. Common on flexible and concrete surfaces
f. Surface Openings	Road Disintegration	➢Loss of road surface ➢Breakup of road into craters with <50% of the road width available to traffic All common on flexible surfaces.
g. Manholes/Ironwork (Flexible & Concrete Surfaces)	Ironwork Level	The level of the ironwork has not been set correctly, or has moved relative to the adjoining pavement level
	Joint Sealing	No joint sealer has been used while installing the ironwork, or the joint sealer has failed
	Failure of Adjoining Pavement, surround	Defect categories a,b,c,d or e in the immediate vicinity of the ironwork

h. Joints	Joint Seal Damage (Concrete Surfaces only)	➢The joints between concrete slabs have widened, or ➢Joint sealant has been removed, or ➢Spalling, where there is a loss of a piece of the concrete pavement from the edges of the joint.

The severity levels - Low, Medium and High - have been replaced with the following:

•Non-Compliance

•Hazard

•Immediate & Serious Hazard - this requires the site to be made safe.

The old severity levels will continue to be shown for defects created before this release.

If an Immediate & Serious Hazard defect is found/reported to an inspector - for which the licensing authority is forced to take action to make the site safe - a fee may be applied by the licensing authority to cover their costs.

The fee will appear on the fees tab as a "Make Site Safe" fee.

In the past, when a breach was reported it remained with a status of open until the Resolve Breach button was selected.

However, in most cases, a breach does not actually require action, so the following changes have been introduced.

•When a breach is created it has a status of Recorded. As before, the applicant will be notified.

•If no direction is issued for the breach then no action will be required.

•If a direction is issued for the breach, the applicant must select the Resolve Direction button and provide a comment/response to the direction given by the licensing authority. If relevant, an image can also be uploaded.

If an Immediate & Serious Hazard breach arises - for which the licensing authority is forced to take action to make the site safe - a fee may be applied by the licensing authority to cover their costs.

When a breach of conditions arises, the inspector can issue a direction to the applicant to take a particular action and/or to require the applicant to provide a response.

In the past when a breach was reported it remained with a status of open until the "Resolve Breach" button was selected.

This has changed with the introduction of Directions.

•When a breach is created it has a status of Recorded. As before, the applicant will be notified.

•If no direction is issued for the breach, no action will be required.

•If a direction is issued, the applicant must resolve it and the inspector can then determine whether they wish to accept or reject it.

To resolve a direction:

1.Select Resolve Direction

2.Enter a comment

3.If relevant, upload an image

4.Click Confirm.
